District Forms

Most of the District's forms for families and the public are available on this website and can be completed electronically.

To assure that forms work correctly:

  • Download the form to your computer (even if it opens in the browser).
  • If it opened in your browser and it does not say "Adobe Reader" on the menu bar, close the form and open it from your computer as follows.
  • Right click (or control + click) on the document and select "open with"; choose Adobe Reader.
  • Be sure that you are using Adobe Reader 9.0 or higher.

If you have trouble opening the form in Adobe Reader, you can print it and complete by hand.
